Primary Care Practices at Two Eskenazi Health Center Locations Are Moving

Spring is approaching, which means it’s the perfect time for a change. Eskenazi Health has built a state-of-the-art, 95,000-square-foot facility, Eskenazi Health East 38th Street, opening on April 17. The primary care practices at Eskenazi Health Center Forest Manor and Eskenazi Health Center North Arlington will be moving there. All the primary care providers and staff at those locations will be making the transition, so if your care is already with us, your medical team won’t change.
What will change is how many additional health services will be available to residents on the east side of Indianapolis and along the Northeast Corridor. The new location has so many offerings the current two locations do not. In addition to primary care, the new building will provide access to Sandra Eskenazi Mental Health Center services along with podiatry, chiropractic services, physical therapy/rehabilitation services, imaging services, a pharmacy, lifestyle medicine, financial counseling and so much more. If you have primary care with us, you will no longer need to travel to a separate facility for so much of your health care.
In the meantime, here’s what Eskenazi Health Center patients living on the east side of town or along the Northeast Corridor need to know:
Eskenazi Health Center Grassy Creek and its providers are staying where they are; their patients won’t move.
For patients with primary care at Eskenazi Health Center Forest Manor or Eskenazi Health Center North Arlington, any appointments scheduled on or after April 17 will be at the new facility, located at 6002 E. 38th St. (Don’t worry if your appointment initially includes the previous location in Eskenazi Health MyChart.) Those with appointments after the move will hear from an Eskenazi Health Center team member before the appointment to confirm details and answer questions.
The new facility is located at the northeast corner of East 38th Street and North Arlington Avenue on the new IndyGo Purple Line, which should be completed this fall. For routes from your home, go to www.indygo.net/routes. There will be free parking at Eskenazi Health East 38th Street.
For patients with pharmacy prescriptions during this transitional period, just access our free delivery service by calling 317.880.4500.
If you’ve signed up for an Eskenazi Health MyChart account to keep track of appointments, provider communication and prescriptions but have run into issues with it, call 317.880.2273.
Those who have questions or concerns about the move can call 317.880.7666.
