Eskenazi Health is one of the first health systems in the country to utilize the new Guiding an Improved Dementia Experience (GUIDE) model for dementia care.
The GUIDE model increases access to high-quality care by addressing the needs of unpaid caregivers of those living with Alzheimer’s disease and related dementia.
Caring for those with dementia can cause significant out-of-pocket costs and financial strain, which GUIDE helps to mitigate.
The model was announced last year by the Centers for Medicare and Medicaid Services (CMS) Innovation center, who worked, in part, with providers at Eskenazi Health to guide the payment model.
“The magic comes from the care team who works with the patient outside the office, in the home, in the community, with the caregiver,” said Dr. Malaz Boustani, director of care innovation at Eskenazi Health. “Through decades of research in brain health, we have learned that by reorganizing care, we can obtain better outcomes at lower costs.”
GUIDE provides support and resources for caregivers to reduce strain and keep patients in their homes, delaying nursing home placement. It became available at Eskenazi on July 1.
The model stems from President Joe Biden’s executive order to increase access to high-quality care, support caregivers and address Alzheimer’s disease.
The model will recruit patients enrolled in Eskenazi Health Senior Care programs and Eskenazi Health patients who are 65 years or older and not part of Senior Care.
